Kokomo School Corporation officials take all alleged bullying incidents seriously and want parents and students to have access to bullying report forms. Bullying is defined as “an overt, unwanted, repeated act or gesture, including verbal or written communication or images transmitted in any manner, physical acts, or any other behaviors that are committed by a student or group of students against another student with the intent to harass, humiliate, intimidate, or harm the targeted student and create for the student an objectively hostile student environment.”
Please understand a one-time incident is not considered bullying. Such incidents should be reported to your child’s teacher and/or principal.
Forms are below that include a parent/guardian (alleged) bullying report form, a general (alleged) bullying report form, a student (alleged) bullying report form for elementary students, a student (alleged) bullying report form for middle school and high school students, a staff (alleged) bullying incident form, and a (alleged) bullying report and incident form for students/parents & caregivers/teachers & staff/volunteers.
